* When the ovarian cyst persists for more than 3 menstrual cycles.
* When the cyst appears in a postmenopausal woman.

For both cases, an ultrasound, laparoscopy and ovarian biopsy should be considered especially if a family member previously had ovarian cancer. A blood test to detect a tumor indicator can be indicated but many cases of false positives are common.
